The Parties agree to the following for consideration received:
PUPS & MUTTS FACILITIES
Pups & Mutts is a dog day care and boarding facility. Pups & Mutts provides an environment for dogs to socialize, play, romp and run in supervised and closed off quarters. Both indoor and outdoor facilities are provided to ensure the dogs are kept in closed off and safe play areas. Pups and Mutts reserves the right to immediately change your pet’s type of boarding or daycare if we believe it is necessary to protect the health and well-being of your pet, other pets and our staff and owner agrees to pay any additional costs associated with the change.
HOURS OF OPERATION
Day care hours of operation:
Monday- Friday: 6:30 am to 6:30 pm
Boarding check in/check out hours:
Monday- Friday: 6:30 am to 6:30 pm
Saturday: 8:00 am to 10:00 am & 4:00 pm to 6:00 pm
Sunday: 8:00 am to 10:00 am & 4:00 pm to 6:00 pm
Lobby is closed for New Years Day, Easter, Memorial Day, 4th of July, Labor Day, Thanksgiving, and Christmas. Day care will not be offered during these days. No boarding drop offs or pickups are available these days.
ARRIVAL AND DEPARTURES
Day care is offered from 6:30 am to 6:30 pm. A late pick up charge of $10.00 is incurred after 6:30 pm. No pickups are allowed after 7:00 pm; an automatic overnight stay will be required and an additional $35 fee is charged in addition to the day care charge.
Boarding drop off and pickups are available during the posted boarding hours of operation. Boarding rates are based on a 24-hour (or less) cycle. Late pickups will be charged as daycare fees.
Pups & Mutts requires a credit card to confirm reservations. The balance is payable at check in. A valid credit card number will be maintained on file. We accept Visa, Master Card, checks and cash. All prices are subject to change without notice.
Pups & Mutts requires 24 hour notice to cancel reservation for non- holiday reservations for a full refund.
Holiday reservations require cancellations a minimum of 72 hours prior to scheduled date of arrival for a full refund.
There will be NO refunds if cancellation occurs less than 24 hours for non-holiday reservations and less than 72 hours of holiday reservations. Any “No Shows” will not be issued a refund. No refunds will be issued if you pick up your pet earlier than scheduled.
No refunds are issued if your dog leaves Pups & Mutts before his scheduled departure date due to behavioral or health concerns.
PETS NOT PICKED UP ON DEPARTURE DATE
In the event that the Owner or emergency contact does not pick up the pet on the agreed upon departure date, the Owner hereby authorizes Pups & Mutts to continue to provide daily services as set forth in the agreement at the Owner’s expense. If such pet is deemed abandoned under state laws, Pups & Mutts will use its discretion permitted by law to follow the Abandoned Pet Procedure.
ABANDONED PET PROCEDURE
We will attempt to contact the Owner by telephone and/or in writing using the information provided, advising the Owner that if the pet is not picked up within a reasonable time period, the pet will be deemed to be abandoned and that Pups & Mutts will deliver the pet to a third party adoption partner, animal shelter, or other similar government agency. If the Owner fails to pick up the pet for any reason, THE OWNER RELEASES PUPS & MUTTS FROM ALL FURTHER LIABILITY AND RESPONSIBILITY FOR THEIR PET.
Proof of current vaccination is required. Each pet’s vaccination certificate as well as any other relevant information regarding the pet(s) must be maintained on file. Only veterinarian verified vaccination records are accepted.
Required vaccinations for dogs: Bordetella (kennel cough), Rabies, DHLPP (Distemper, Hepatitis, Parvovirus, and Para influenza).
Dogs must be spayed or neutered by 7 months of age and have Bordetella Vaccination (5) days prior to any stay with Pups & Mutts.
Dogs are required to be on a flea and tick preventative.
Please feel free to bring comfort items for your pet. However, do not bring items that are valuable as Pups & Mutts will not be held responsible for possible loss or damage to any personal item or toys left with the pet.
PHOTOS AND VIDEOS
Pups & Mutts will take pictures and/or video of all dogs during their stay at Pups & Mutts. The pictures and video will be used in media, marketing, advertising, illustration, trade or promotional materials.
REFUSAL OF A PET
Pups & Mutts, to the best of its ability, practices safety for the health of all dogs and staff. Pups & Mutts strongly relies on the Owner’s promises and pledges to Pups & Mutts regarding dogs’ health and behavior. Safety is top priority at Pups & Mutts and during all times of this agreement, Owner understands that Pups & Mutts will not accept dogs with the following:
a. Dogs less than 3 months of age
b. Expired vaccinations
c. Known aggressive tendencies towards other dogs or humans
d. History of one or more dog bites
e. Ticks and/fleas (dogs are inspected at intake, if fleas/ticks are found; dogs are given flea/tick treatment at owners expense).
f. Un-altered dogs older than 7 months
In the event of an emergency, all effort will be made to reach the Owner and coordinate proper care. If the Owner cannot be reached, Pups & Mutts will request emergency care from the pet’s veterinarian if possible, otherwise from one of our veterinarians on call. The Owner will be responsible for all charges incurred for all treatments, medication, or necessary surgery to your pet(s). Pups & Mutts will charge the Owner for any costs related to emergency care and treatment of your pet(s).
I acknowledge that my emergency contact below is aware they have been chosen, is aware of my pets stay at Pups & Mutts, and is willing to pick up my pet in the event that my pet must leave before its scheduled departure day and will take responsibility for my pet in my absence.
ASSUMPTION OF RISK
The Owner acknowledges and is aware that the employees of Pups & Mutts are not veterinarians, do not have backgrounds in animal medicine and are not expected to diagnose or to detect illnesses, health problems or allergic reactions regarding the dog(s) staying at or visiting Pups & Mutts. In addition, the Owner acknowledges that vaccines, although efficient, do not necessarily protect your pet(s) against all contagious illnesses that may affect your pet at Pups & Mutts. The Owner acknowledges and agrees that all activities, play time, and outings will be conducted in the presence of attentive and competent staff, ensuring the safety of all pets. All the necessary precautions will be exercised to ensure the safety of all involved; however, it is possible that injuries or incidents causing harm can happen. Pups & Mutts or its staff will not be held responsible for any injuries sustained by your dog(s). The Owner also acknowledges and agrees to assume all risks for illness, disease, reactions, or incidents causing harm that may occur to your dog by allowing your dog to stay at or visit Pups & Mutts. The Owner also assumes all risks of damages, injuries or death caused by your dog(s) to other dog(s), visitors, or Pups & Mutts employees or to any Pups & Mutts assets and that Pups & Mutts shall have no liability for any harm to such dog(s) or damages involved.
You represent to us that you are the Owner(s) of the pet and that you are fully authorized to enter into this agreement. All the information about you and your pet in this agreement is true, accurate, and complete. To the best of your knowledge, your pet has no illness, injury, or behavior problem (including aggressive or biting behavior) that has not been disclosed to us. You agree to indemnify and hold us harmless, from and against all loss, damage or expense, including legal fees resulting from misrepresentations by you or your representatives or resulting from your pets stay.
